The New York Times is proud to announce its 2011 Tribute to Nurses Program. This special section, which will publish on December 4th, 2011 in the New York Times Magazine, will help to bring attention to the field of nursing by recognizing several nurses' contributions to the field. This year, we are soliciting nominations from those people who know these nurses' abilities best: those they work with everyday. To ensure we recognize the most outstanding in the field, we need your help. During 2011 National Nurses Week, several nurses from your hospital were likely recognized by your organization for their exemplary on-the-job performance. In order to recognize these nurses on a broader level, only nurses who were nominated for an award during their hospital's National Nurses Week recognition programs are eligible for a 2011 Tribute to Nurses award. As part of the submission process, we will require that you, the nominator, provide documentation that shows he/she was nominated for and/or won an award during National Nurses Week 2011. Please complete the form and attach the nurse's application/write up for 2011 National Nurses Week no later than October 15th, 2011. Please note that the following guidelines exist for 2011 Tribute to Nurses:

Within each category, The New York Times, along with the Tribute to Nurses Editor, will select one winner, one finalist and two honorable mentions. This number is subject to change based on number of submissions.
